In order to complete your Practicum requirements you must:
- complete a minimum of 160 hours of direct practicum work
- you may use up to 40 hours for preparing the report; prepping for and completing the defense, including any changes to the report required by the practicum committee at the conclusion of the defense
Practicum Grading Rubric
Fill out one of these forms for each committee member. Fill in the upper portion with your name, one of your committee member's names, and the type of committee member he/she is: supervisor, site mentor, or member. Also, insert your objectives on the left side of the form. If there are more than 5 Learning Objectives, add additional rows to this form. Bring these forms to your Practicum defense. If a committee member is attending remotely, send them their form electronically for completion during your defense. Each form should be signed (electronically if remotely attending) and given to the School of Public and Community Health Sciences (SPCHS) Administration office.
Practicum Site Mentor Evaluation
This is the form your site mentor should fill out. Please tell him/her to email or deliver it back to the School of Public and Community Health Sciences (SPCHS) Administration office once it is completed.
Fill out this form through Project Summary/Abstract, sign and date at Certification of Hours and bring this form to your Practicum defense. This is the form your committee members will sign once you have finished your presentation. Once signed by all the committee members, this form is given to the School of Public and Community Health Sciences (SPCHS) Administration office.
Student Practicum Site Assessment
Fill out this form evaluating your site mentor and send it to the School of Public and Community Health Sciences (SPCHS) Administration office.

Every MPH student is required to maintain and complete a Portfolio during their Master of Public Health program. Due to the cumulative nature of the portfolio, you can produce a more comprehensive product by beginning compilation at the start of your program and continuing as you progress through the program.
Portfolio Review Committee Form
This form should be completed prior to working on your portfolio defense. You will list the three members you would like to serve on your Portfolio Review Committee. You would then route this form to all the members you have identified and ask them to sign the form if they are in agreement to serve on your Portfolio Review Committee. Once completed, this form should be turned into the School of Public and Community Health Sciences administration office.
Portfolio Defense Grading Rubric
A separate form should be filled in at the top for each of your Portfolio Review Committee members. Fill in your name, the date of your portfolio defense, the committee members name and their type (chair, faculty member, alumna, etc.). On the day of your defense, give this sheet to each of your committee members. At the end of your defense, ask each committee member to complete and sign the form and turn it into the School of Public and Community Health Sciences administration office.
Post-Assessment for Students
After your portfolio defense is over, complete this Post-Assessment for Students form and turn it into the School of Public and Community Health Sciences administration office. This form is needed for our continuing accreditation with the Council on Education for Public Health.
Practicum/Portfolio Defense Checklist
Our accreditation agency (The Council on Education for Public Health - CPEH) requires all MPH defense documents be retained for review during CEPH site visits in either printed or electronic format. As storage space for the MPH program is limited, you are required to submit both draft and finalized copies of your Practicum and Portfolio documents in e-format.
All Practicum and Portfolio defenses may be recorded and stored in your file. These recordings are intended for educational use by current MPH students preparing for their defenses. These recordings are not released for viewing to current MPH students without express written permission from you.
